Two Defensive Visits in the Same Inning?

After walking the first two batters of the inning, the coach of the defense requests "time" to visit his pitcher. This was the coach's first visit of the game. Several batters later, with a runner on third base, the defensive coach again requests "time" to talk to the same pitcher to talk about defense strategy.

During the visit, the offensive coach reminds the plate umpire that this is the second visit to the same pitcher in the same inning, so that pitcher must be removed. The defensive coach objects.

What does the umpire do now?

In NF, there can be more than one defensive conference to the same pitcher in the same inning without penalty -- as long the other criteria mentioned in NF 3-4 are met.

In the situation below, the coach's second visit to his pitcher was the second charged visit of the game, so the pitcher does not need to be removed.
