Hit by Pitch while Swinging

With no outs and a 3-1 count on the batter (B1), the runner on second base (R2) is stealing on the pitch. B1 swings hard at the pitch, but the pitch strikes the back of B1's hand while he is swinging. The ball rolls a few inches in front of home plate in fair territory. The catcher picks up the ball and tags B1, but then the catcher's throw to third base is not in time to retire the stealing R2.

Who is out and who is not out on this play? Where are B1 and R2 placed?
