Charged Offensive & Defensive Visits


 
Tie game, no count on the batter, runners on 1st and 3rd base, and no charged defensive visits to the pitcher thus far in the game.
 
The defensive coach requests "time" to visit the pitcher and the infielders to discuss how they are going to handle this "1st and 3rd situation". When play resumes, the pitcher throws to 1st base in an attempt to pick-off the runner, but the runner advances safely to 2nd base -- so now we have runners on 2nd and 3rd.
 
The defensive coach now requests "time" and wants to visit the pitcher and infielders to discuss how to handle an apparent bunting situation. Can a defensive coach have two charged visits without his pitcher throwing a pitch?
 
BONUS QUESTION: Under the same circumstances, what if it is the third base coach that requests "time" to talk to his runners that are on 1st and 3rd base -- but then the third base coach wants "time" again after the runner from 1st had safety advanced to 2nd. Is a second offensive visit allowed without a pitch being thrown?

There is not a limitation on the number of charged defensive visits. After three charged visits, the pitcher must be removed after each subsequent visit. The offense can only initiate one visit per inning. Requests after the first charged offensive visit of the inning should not be allowed.
