Pickoff Thrown Out of Play

The runner on 1st base and is stealing on the first move by the pitcher. The pitcher's throw over to 1st gets past the first baseman and goes out of play. Where do you place the runners in these situations:

A. A right-handed pitcher steps behind the rubber before throwing to 1st?

Call
Since the pitcher has disengaged from the rubber, his throw to 1st base is considered as any other infielder -- so the runner is awarded 3rd base.

B. A left-handed pitcher stays in contact with the rubber during the throw to 1st?

Call
In both B and C, the pitcher is still in contact with the rubber, so the award is one base (2nd base).

C. A right-handed pitcher uses the "jump turn move" (simultaneously disengaging the rubber and throwing) during the throw to 1st?

Call
In both B and C, the pitcher is still in contact with the rubber, so the award is one base (2nd base).

Bonus questions D, E, and F: What is the call on the three situations above if the pitcher makes the moves described but then fails to throw the ball to 1st base?

Call
For question D: Since the pitcher is off the rubber, he need not throw to first -- so no penalty if he does not throw.

For questions E and F: it is a balk if the ball is not thrown to 1st base while in contact with the rubber.
