Tracking Defensive Visits


 
In NFHS, the defensive coach is permitted 3 charged visits before he is required to replace his pitcher on the 4th charged visit.
 
The situation below is a bit lengthy, but it actually occurred, so I thought it was relevant...
 
Early in the game, "Team A" had a charged visit to the pitcher. Later in the game, Team A's coach again visits the pitcher. This is the second charged visit of the game for Team A. The pitcher then walks the next batter,  so the defensive coach visits again in order to replace the current pitcher with Team A's super-star relief pitcher.
 
After the 8 warm-up pitches by the relief pitch, the offensive team puts in a pinch hitter (to try to counteract the new super-star relief pitcher). However, then Team A's coach returns from the dugout to the mound to talk to the new pitcher about this new pinch hitter. 
 
The coach of the offense really does not want his team to face this super-star relief pitcher, so he raises two interesting issues with the umpires:
 
1) The Team A has visited the new relief pitcher twice without the pitcher ever delivering a pitch.
 
2) This is the third trip to the mound for Team A THIS INNING -- so, coupled with the visit earlier in the game, on the 4th visit, the pitcher should be removed.
 
These are interesting points raised by the coach of the offense. What actions are needed from the umpires?

After the starting pitcher walked the batter, the coach visited the mound to replace the pitcher -- so this is not a charged visit. When the coach returned to talk to his relief pitcher about the pinch hitter, it is the first visit to his relief pitcher and the third charged visit for his team.
 
The umpire should remind the coach that this is his 3rd charged visit, but there are no other actions.
